Description
Job Purpose: Under the general supervision of the Director of Home Care and in accordance with Visiting Nurse Services of Iowa standards, practices and policies, and Medicare and Community Health Accreditation program (CHAP) standards and regulations, provides health promotion and health prevention activities and home health nursing services for individuals, families, groups, and communities in a variety of settings.
Education: Completion of an accredited registered nurse program.

One year of clinical experience in an acute care, long-term care or home health setting preferred.

Licensed to practice nursing as a registered nurse in the state of Iowa.
Qualifications: Knowledge of public health nursing principles and practices, including community health issues, preventive measures and community resources.

Possess excellent verbal and written communication skills using the English language.

Possess excellent critical thinking and decision making skills as well as strong nursing assessment skills.

Keeps current in nursing knowledge and skills.

Ability to plan, organize, teach, and work collaboratively with individual clients/families, community groups/organizations, and other health care providers to achieve public health objectives.

Ability to exercise independent nursing judgement and work well within a team setting.

Ability to provide services in various community settings.

Ability to communicate and work effectively with diverse populations.

Ability to work with the management information system, enter client data appropriately and assist with data collection and reporting.

Maintain CPR certification.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Appearance and personality appropriate for daily contact with patients/clients, social workers, community agencies, and co-workers.
Responsibilities: Assesses the population served. Gathers data from a wide variety of sources. Contributes as appropriate to other agency and community health assessments.

Plans for services to individuals, families, groups and communities. Participates in agency and community planning activities as directed. Confers with other agencies and providers in order to collaborate and coordinate efforts.

Provides services, which may include but are not limited to physical assessment, education, case management, referral, advocacy, screening, follow up, and consultation.

Follows accepted policies/procedures to assure safety for self and other staff and clients.

Participates in marketing and outreach activities, and represents the agency in community groups and activities as directed.

Maintains accurate and timely documentation of all services. Contributes to appropriate statistical data collection for billing purposes and completion of reports and documentation in the time specified by supervisor. Documents accurate, reimbursable, comprehensive, goal directed nursing notes.

Develops a comprehensive plan of care. Establishes realistic, achievable, measurable and quantifiable client goals.

Performs OASIS/Comprehensive Assessment of clients within regulatory timeframes.

Obtains physician orders for provided services, treatments and changes in frequency and duration of services provided.

Keeps clinical records reviewed and updated including medication sheets, care plans, and progress notes.

Plans for discharge at the initial visit. Completes required documents for preparation of discharge of the client.

Participates in evaluating the effectiveness of program activities.

Accepts and supports the mission and vision of the agency.

Seeks and obtains sufficient information necessary to function effectively in the health care setting where assigned.

Accepts responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

Assists with the orientation and training of new staff.
